An Institutional Special Needs Plan (I-SNP) is a Medicare Advantage plan for people who live in — or are expected to need the level of care provided by — an institution such as a nursing facility, or who need that level of care while living at home.
Who an I-SNP is for
- People who have lived, or are expected to live, in a qualifying facility (such as a skilled nursing or long-term care facility) for 90 days or more.
- People who require an institutional level of care, sometimes provided at home, as confirmed by a state-approved assessment.
How it works
- On-site care coordination. I-SNPs typically bring care management to where the member lives, often with nurse practitioners or care teams who work directly with facility staff, which can mean fewer hospital transfers.
- Included Part D coverage and benefits designed around higher-needs care.
